DescriptionA Prospering Vision (APV) is an Outpatient Behavioral Health Private Practice Agency with locations in the cities of Waterbury, Danbury, and Naugatuck that utilizes an array of therapeutic modalities delivered through an individual, couples, families, and group session format.
APV provides services including Outpatient Psychotherapy, Medication Management, Professional Trainings and Workshops, and an innovative Internship Program for Bachelors and Masters’ Level Students. APV has developed, as well, a Transitional Home with an Outpatient Therapy Program in the New Haven County that works towards providing families encountering homelessness with individualized support and transitional housing to help support sustainability in the community.
At HOH we offer a one-year Transitional Home; Intensive Therapeutic Model; Dialectical Behavioral Therapy; Parenting, Nutrition, Financial Planning, and Employment Preparation.
Since 2015 we have been running a successful Internship Program offering placement opportunities for students training. Our program has been designed for Master Level Counseling and Social Worker students, License Nurse Practitioners (APRNs)’ students, and Bachelors and Masters’ Level Human Services students. We view internship as a three-way partnership between the student, agency, and school, with the goal of providing the student with the ability to learn all areas stipulated in the Learning Contract. Our overall objective is to help the student integrate theory and practice.
An internship with APV provides students with the opportunity to have real life experiences with individuals, families, communities and organizations. Field instruction, provided by experienced and highly skilled certified and licensed Social Workers, determine to help students connect these experiences to their academic training.
We carefully match students with placement sites, and assignments are varied to offer each intern the highest quality learning opportunity and environment. Our aim is to expose students to the range of problems that lead clients to our doors in the many diverse settings APV operates in.
Throughout the Internship Program supervisors evaluate Teamwork, Communication, Client Focus, Attendance and Punctuality, Job Knowledge, and Goal Completion. Our Learning Goals and Objectives Plan for Internship has been meticulously elaborated to ensure, as an outcome, that all students become highly qualified professionals once the program is completed.
ResponsibilitiesSocial Worker & Counseling: Provide family-focused, child-centered, individual, family and group therapy; create individualized treatment plans; create individualized behavioral plans, guide family and team in formulation of treatment goals and strategies; provide crisis intervention; educate families about mental health diagnosis and issues; teach parents new skills to help them cope with their child’s mental health issues; coordinate discharge and aftercare services; complete clear and legible required record keeping in a timely manner; must achieve (X) amount of billable hours each week (X is based on number of clients); participate in Rounds and supervision with Program Director; participate in agency and outside trainings, and workshops; abide by all Department of Children and Families practice standards; must be able to work independently regularly exercising discretion and judgment.
Human Services: Attain CPR and First Aid Certification; provide a supportive environment for women and children; foster opportunities for clients to use strengths and interests in successful ways; support resiliency and determination with the least intrusive and controlling approaches; encourage and model effective ways of dealing with anger and stress; provide structure and predictability to enable a sense of safety; assist with or perform personal care and daily living needs; ability to set professional boundaries and limits; ability to establish a positive rapport with mothers and children; complete clear and legible required record keeping in a timely manner; provide home management services as needed; prepare meals to meet nutritional requirements and/or specialized diets; participate in Rounds and supervision with Executive Program Director; participate in agency and outside trainings; and workshops; must be able to work independently regularly exercising discretion and judgment.
Advance Practice Register Nurse: Analyzes and interprets history, including presenting symptoms, physical findings, and diagnostic information to develop appropriate differential diagnoses; diagnoses and manages acute and chronic conditions while attending to the patient’s response to the illness experience; prioritizes health problems and inter; employs appropriate diagnostic and therapeutic interventions and regimens with attention to safety, cost, invasiveness, simplicity, acceptability, adherence, and efficacy; formulates an action plan based prioritizes, coordinates, and meets multiple needs and requests of culturally diverse patients; uses sound judgment in assessing conflicting priorities and needs; builds and maintains a therapeutic team to provide optimum therapy; obtains specialist and referral care for patients while remaining the primary care provider; advocates for the patient to ensure health needs are met; consults with other health care providers and private/public agencies; incorporates current technology appropriately in care delivery.
