Description

Providing one-to one or up to one-to-four staff to client ratio job training for workers placed into community integrated supported employment work sites. Develop effective communication – work cooperatively with community employers, employees, clients and other supported employment program staff.

Responsibilities

Training Responsibilities:

• Systematically instruct clients in skills to maintain job and achieve objectives in Individual Habilitation Service Plan (IHSP).
• Maintain client intervention sheets including case notes; prepare monthly reports; prepare and update IHSP (semi annual and annual report); maintain client sub notes.
• Develop systematic task analysis.
• Provide data as requested on specific goals and objectives in monthly casenotes and monthly reports.

Vocational Responsibilities

• Maintain quality control and safety standards on the job sites.
• Ensure all employer policies and procedures are followed to employer standards.
• Responsible to maintain production records for assigned clients, time sheets or assist clients with employers timecards.

Community Responsibilities

• Train clients in skills to support their independent living as it relates to their job status.
• Provide social skills and mobility training as needed.
• Keep emergency numbers and procedures updated for all clients in community jobs and classes.
• Promote good public relations in the community both for NVSS and clients in community training.

Behavioral Responsibilities

• Inform Employment Specialist in writing when behavior problems arise.
• Assist in the development, implementation, data and follow through of client behavior management plans.

Safety/Transportation Responsibilities

• Be aware of safety procedures and location of safety manual.
• Immediately inform management staff if an unsafe situation occurs or is observed.
• Document special incidents on the proper forms as outlined in Special Incident Report, (SIR) procedures.
• Immediately inform management when client health issues arise.
• Keep job site notebook current with client emergency numbers and procedures, current task analysis and job site safety sheets.
• Immediately inform management staff if an unsafe situation occurs or is observed.
• Research and provide means of transportation to community training sites; (e.g., City Bus, NVSS van, instructor vehicle).
• Ensure NVSS vehicles are clean pre and post trips and communicate to program assistant if not.
• Conduct a pre trip safety inspection of all NVSS vehicles.

Requirements

CAL DRIVER'S LICENSE
VEHICLE W/ 3 - 4 PASSENGERS
NO FELONY OR MISDEMEANORS
PASS PHYSICAL AND DRUG TEST
