Description

Responsible for providing case management services and case coordination
for women and children in the Transitional Housing
Program. This person must have or be willing
to develop working knowledge of homeless issues, oppression, client advocacy,
community resource, drug and alcohol issues, and human development. The
work schedule may require some evening and weekend hours.

Responsibilities


Direct Services
1. Provide case management and maintain a case load of 3-4 participants in Phase I or Phase
II of the program
2. Document all client and family information, including data required by the evaluation team.
3 Develop and complete case assessments and service plans with each family in care.
4.Place participants in transitional housing units and provide ongoing supervision for each
participant.
5.Provide referrals to and coordinate services with adjunct services as required, including
assisting with employment and training/placement, school enrollment/attendance,
health care/mental health services, providing
and/or coordinating transportation to residents,
and coordinating childcare and other support
services.
6.Facilitate group or individual meetings to pass on critical skills and provide support for program
participants.
7.Schedule, coordinate and facilitate team meetings
with all involved agency representatives and
others who are involved with the participants support.
8. Counseling Families and youth with behavior management and solution focussed techniques to facilitate
personal growth and minimize crisis.
9.Complete discharge summaries, implement after/care plan and provide follow-up services if
needed once families are discharged.

Requirements

Master’s degree in behavioral sciences or related field and two years' experience
working in case management or Bachelor’s degree in behavioral science or
related filed and four years' experience working with youth. Experience with high risk populations involved with juvenile justice /dept of corrections and/or
CPS/Family Court preferred. Experience in
community networking,case coordination for multi disciplinary teams providing a safe and nurturing
environment for youth and demonstrated organizational skills required.
Experience/ knowledge of homeless issues, client advocacy, community
resources and other high risk populations desired. Must be sensitive and
responsive to the cultural differences in the organization's service population.
