The Emergency Department (ED) Case Management Clinician leads a multidisciplinary team across the continuum of care to initiate, implement and coordinate all aspects of the utilization management and discharge planning function from admission through post-discharge for an assigned patient population. The ED Case Management Clinician assures that every patient admitted to Glendale Adventist Medical Center via the ED during his/her duty hours is screened for admission appropriateness, and is referred to a House Supervisor for bed assignment in the most appropriate nursing unit and level of care. Minimum two years clinical experience. ED or critical care and prior Case Management experience, in ED preferred. Graduate from an accredited nursing program. Basic computer skills in Word, Excel, and Power Point required. Bachelor's degree in nursing preferred. Must have a current California RN license.
