Celebrate Life Cancer Ministry (CLCM) is a non-profit, 501c3, multicultural health organization for all cancer types. The ministry promotes and sustains the quality of life after cancer diagnosis through spiritual encouragement, education, and financial resources to the cancer fighter, survivor and their families.
CLCM is in the process of developing a structured Patient Navigation program for all cancer types. This project will include assisting on the ground level in developing and evaluating the process. Communicating with as assisting patient remove barriers to health care. www.celebratelifecm.org
Responsibilities
- Provide customer service in hospital, helping family members, visitors, and caregivers
- Understand relationships among healthcare personnel, in both hospital and clinic setting
- Offer research skills on multiple healthcare issues, resulting in presentation or publication
- follow up with patients with suspicious findings
- help streamline appointments and paperwork for patients
- conduct focus groups and record the findings.
- link patients, caregivers, and families with needed follow-up services
- keep in touch with patients while they are receiving care and track their progress
Requirements
- Prefer student with experience in healthcare delivery system
- Perform well in environment that values compassion, helpfulness, patience, and resourcefulness
- Compassionate person who is flexible, persistent, patient, and can work with people of all social economic levels.
- Has a good understanding of the issues of financial, cultural, educational and social barriers to information and care.
- Ability to communicate effectively with patients , their families and health care providers.
