Plan, develop, and coordinate hospice referrals for patients. Serves as an educational resource to healthcare facilities and the community regarding hospice care services. Plans, implements and conducts educational hospice programs. Collaborates with Director of Program Development to plan and develop ongoing strategies.
Professional Registered Nurse with current New Jersey license. Baccalaureate degree preferred. One-year experience in Public Health Nursing and one-year experience in hospital setting preferred.
