This position serves as the clerical support to the intake process for the Sutter Visiting Nurse and/or Hospice home care and hospice locations. This position assists the Skilled Nurse Intake Specialist in ensuring the effective communication between Home Care Coordinators, intake staff, referral sources, physicians and the SCAH organization. Referral source interactions include, but are not limited to, working closely with the SCAH Home Care Coordinators, physicians, board and care facilities, other referral sources, patients, SCAH management and other staff and the general public. The Intake Specialist receives general referrals from external sources for nursing therapy services, prescreens new referrals and initiates the information gathering process into the patient information database, Horizon, the information into the billing system, as well as initiating and maintaining new files and associated duties.
