Senior Connection is a Home Health Care Company that specializes in coordinating housing for local Hospitals and Gov. Agencies for patients who are discharged from the hospitals, who need continuing care . The Sr. Housing Coordinating Position requires students to work independently, while finding appropiate resources for patients, including housing and setting up wrap around programs that meet the patients needs after being discharged. Interns will work many hours at Sr. Connections local facilities, along with working from home. This Internship program will provide students case management and discharge planning training. Great for students who are attending school for a Case Management or Social Work. Students will received a bi-weekly stipend of $400 or more. Hours are based on the students work load and hours needed.
Students and Interns will assist our Case Managers with all cases and will help assist patients with their care needs who will been placed in any of Sr. Connections providers facilities. The daily task for all Interns will vary based on the workload needed. Interns will contact various facilities and assist the social workers and care management team with discharge services and patient care needs. Students will be required to email and fax press releases and memos to all attending agencies daily.
Students must be self motivated and able to complete assisgnments without total supervision. Students who are eligible are required to attend school for any of the Health Care professions or a career in Social Sciences. Students must be computor literate and have access to a computor or laptop.
