DescriptionIntake Counselor takes information from a patient or client before treatment or services are administered
Responsibilities-Schedules initial assessments with consumer and/or family.
-Collects data about individual through motivational interviewing, case history, and/or observational techniques;
-Creates and maintains any and all aspects of the medical chart/file(i.e. registration forms, assessment/evaluations, progress notes, signed intake forms, clinical evaluations, Correspondence, social history, etc.).
-Consults with other legal and treatment agencies and individuals in relations to client records/medical chart.
-Participates in the review of assessment issues, policies, and procedures.
-Participates in training sessions and workshops to enhance intake/assessment/interview skills, treatment planning, and discharging.
RequirementsCompletion of high school or general equivalency diploma, AND one of the following:
-Completion of 30 semester hours at an accredited institution of higher learning, or
-One year of full-time/part time work experience which involved contact/interactions with individuals and families as a regular part of the job.
