Description

•      Coordinate with Program Manger to determine research objectives

•      Develop given research assignments

•      Utilize various sources to find and extract pertinent information

•      Compile and organizes findings in graphs, charts, and diagrams

•      Document, report, and present research findings to Program Manager

•      Search and identify community resources appropriate to meet clients’ needs as identified by reentry care plan and facilitate communication with those resources

•      Enter identified resources into an online database and update it weekly

•      Assist Program Manager and Community Health Worker to submit weekly and monthly case management documentation as required

•      Compose and process various types of correspondence to include letters, sensitive and confidential client information, and other client documentation

•      Travel as required to meet client needs and program guidelines

•      Other duties as assigned
