Key Accountabilities
- Conducting initial assessments within 24 hours
- Coordinating discharge plans
- Managing a caseload of 20-23 patients
- Collaborating with a multidisciplinary team
- Participation in care conferences and interdisciplinary activities
- Comprehensive documentation and adherence to quality care standards
Required Skills

Requirements
- 1+ year of acute care experience
- CA license (or pending license upon start)
- Certifications such as BLS, LCSW, MSW
Work Environment
- Working with diverse patient diagnoses and age groups
- Mandatory weekend availability
- Flexibility to float between facilities
