• Assess patient priorities through thoughtful interviews and accurately document case activities to ensure high-quality care delivery.
• Collaborate with medical teams, families and external agencies to guarantee patients receive fully supported and compliant treatment plans.
• Build strong relationships with community health and social organizations to connect clients with vital local resources.
• Guide the discharge planning process and coordinate post-care services to secure safe, seamless transitions for patients returning home.

Licensure / Certification / Registration

• Required Credential(s):
• None Required.
• Preferred Credential(s):
• Certification. Type 73 license through IL State Board of Education LBS I with endorsement in Transition preferred.
• Licensed Professional Counselor credentialed from the Illinois Department of Financial and Professional Regulation.
• Licensed Social Worker credentialed from the Illinois Department of Financial and Professional Regulation.
• Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation.

Education

• High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.

Work Experience

• 3 years of experience preferred.
