Each applicant must have two forms submitted on their behalf (both found on the BWC website under Reference Forms):

(1) Church Membership Status Form – This form must be filled out and submitted by the Session of the church at which you are a member confirming the status of your membership.

(2) Pastoral Reference Form– This is a reference and recommendation form that should ordinarily be submitted by the session or pastor of the church at which you are a member. However, if you have not been regularly attending and/or in contact with your home church session/pastor (e.g. due to having recently moved or to being away at college), then please have your closest spiritual leader (another pastor, elder, or campus ministry leader) fill out this form.
