Job Summary
New York, New York Nursing Supervisor (RN Supervisor). The organization is seeking an Associate Director of Nursing to plan and develop nursing service, educational programs, and transitional care initiatives aimed at improving patient care and employee performance. This role will monitor performance, support corrective actions, and help coordinate care for a high-risk population to reduce 30-day readmissions following discharge. Candidates are required to bring their license on the first day.
Job Details
Schedule: Days 7.5 shift hours, with a minimum of 37.5 hours per week.
Required Qualifications
- Registered Nurse (RN) licensure
- Bachelors degree in Nursing from an accredited nursing program
- Primary Source Verification of license and registration of Registered Professional Nurse from NYS Department of Education (NYSED)
- Basic Life Support (BLS)
- Minimum 2 years of experience in discharge planning
- Minimum 2 years of experience in utilization review
- 7 years of experience total, with 5 years in a supervisory role
- Crisis management and clinical oversight skills as demonstrated through discharge planning, utilization review, care management, supervisory experience, and patient care improvement
Preferred Qualifications
- Masters degree in Nursing
Additional Information
Provides Associate Director level supervision to the care management department and oversees Accountable Care Managers (RNs) and Care Coordinators (CLWs), along with clerical and ancillary staff. Participates in developing policies and standards of nursing service and education for transitional care within the care management division. Organizes and provides oversight of care coordination to support optimal health in the community and safe return after discharge from inpatient medical/surgical units and the Adult Emergency Department. Performs Director of Nursing duties in his or her absence. Develops and maintains a record-keeping system to document medication errors and counseling.
